medically necessary.
4 Levels of Care
- Routine Home Care is possible as long as the patient’s symptoms are under control. The hospice team supports the caregivers in providing this level of care in the home setting, whether this is private home or an assisted living or long term care facility, such as a nursing home.
- Continuous Care is necessary in the event of a medical of psychological crisis; 24 hour care is provided in the home.
- Inpatient Respite Care occurs when caregivers need to take a short break to maintain their own health. In this instance, the patient may be transferred to the one of many nursing and rehabilitation centers that the Legacy Hospice team contracts with for this service. The stay can be up to five (5) days while the caregiver takes a short break.
- General Inpatient Care is required when symptoms are uncontrollable in the home setting. The patient can be moved to the inpatient care facility that Legacy Hospice contracts with for this service.
